Hello! Welcome to DeForest Area School District’s New Student Online Enrollment (NSOE) – account request page. Please complete all required fields to request an account for access to the NSOE Portal. Once your request is submitted from here, email will be sent with NSOE Portal login details. In the NSOE Portal, you will be able to complete the NSOE Application and submit all documents (see below for the list of documents). New Student Online Enrollment is the first of two steps. Upon approval of the NSOE application, an email will be sent with instructions on the 2nd and final step: Registration. Documents Needed for DASD Enrollment: To help ensure an efficient application approval, it is very helpful to have all the documents that are listed on hand for quick uploading during the application process. The most important documents to upload are: Proof of Residency, Child's Proof of Age, Parent/Guardian ID and Custody Docs (if applicable). 1. Child’s Age Verified (original of one of the following presented): a. Birth Certificate - You can obtain a legal birth certificate from the Register of Deeds in the county that your child was born in. WI Register of Deeds by County b. Current Passport c. State ID or Driver’s License 2. Parent/Guardian Identification: verify guardianship for school enrollment a. Driver’s License (front side only) b. State Identification Card (front side only) 3. Proof of Residency (One of the Following) a. Lease Agreement with current address, names of people residing at the address, effective date, and all parties’ signatures. b. Current Utility Bill (within 45 days); gas, electric, cable, or city/village water with name and current address. c. Home Purchase Agreement or Offer to Purchase with the name of buyer and seller, current address and closing date. - A Property Deed or Land Purchase Agreement is not an eligible document. A habitable structure to reside in must be constructed on the land for residency to be considered. d. Exceptions may be made for a homeless student (as defined in the McKinney-Vento Act). If you need a copy, please go to the Wisconsin Immunization Registry. c. Here are the immunization requirements 5. Legal Guardian Document (only if applicable) a. If you are not the biological parent we will need custody or guardianship documents. |
